Yeah. Thin end of the wedge
The old not adjust for inflation trick? In 20 years someone on the average wage will be caught in it?

We’ve seen it with tax brackets creep; brightline (2 to 10 years); GST (10 to 15%), etc, etc.

That’s why we can’t let wealth or capital gains taxes come in. Sell it as hammering the top 1% and before we know it they will be after the tax on our home!

“Having mistakenly assessed a thruster control failure, standard procedures should have prompted the ship’s crew to check that the ship was under manual control rather than in autopilot. This check did not occur. Remaining in autopilot resulted in the ship maintaining a course toward land, until grounding and eventually stranding.”

There you have it...

For those who don't know, this what drills are for. To test and embed SOPs so they are second nature during conflict. IE the drill for weapon stops firing, is a functional weapons test to ID and rectify. Empty magazine, failure to chamber etc. Its gets more involved the larger the platform and the more people. In this case there would be a drill for losing control of the ship, and I bet the first fucking action is check autopilot. This tells me the core officer group is poorly trained and drilled.

Who's responsible for these poor training standards? Looks like the MCC.

"In the New Zealand Navy, the Maritime Component Commander (MCC) is responsible for the training of the Navy's fleet. The MCC is also responsible for the readiness of the fleet, as well as the specialist teams that support the Navy's mission."

There most definitely is extensive breakdown SOPs and training conducted at least every other day.
Someone just screwed up, plain and simple.
